Form to Request Activity and Program Accommodations
Rowan County School District shall furnish appropriate auxiliary aids and services where necessary to afford individuals with disabilities/impairments, including applicants, participants, companions, and members of the public, an equal opportunity to participate in, and enjoy the benefits of, a service, program, or activity of the Rowan County School District.
Individuals that require an auxiliary aid or service should notify the school at least ten (10) days prior to the event during which time the auxiliary aid or service would be needed.

Meeting Dates for Site-Based Decision-Making Councils

Each school has its own Site-Based Decision-Making council whose membership includes parent representatives, teacher representatives and the school principal. These councils provide parents with the opportunity to work with the teachers and school administration to make decisions related to the day-to-day operations of their children’s schools. All meeting of the Site-Based Decision-Making councils are open to the public.

This booklet does not contain a comprehensive listing of ALL student misconduct and district personnel’s disciplinary responses.  The Rowan County Board of Education, each school’s administration and the site-based council reserve the right to administer other appropriate corrective measures for offenses listed and others that may occur.  Variations from responses listed in the discipline code will be based on the severity of the incident and the number of times a student has had disciplinary referrals.

KHSAA Physical and Parental Permission Form (Middle School)

All students interested in participating in a school sport must have a sports physical. The physical can be completed by a physician, a physician assistant, or a nurse practitioner. The KHSAA Physical and Parental Permission Form must be signed by the physician, the parent/guardian, and the student and then returned to the school.
